Designing for the operational middle: A sociotechnical framework for integrating psychosocial risk into aviation human factors and safety management.
WHAT?
Paper arguing that although aviation Human Factors and Safety Management Systems (SMS) have become effective at managing technical failure, procedural compliance and crew coordination, they do not account for the everyday psychosocial conditions that influence operational performance.
Psychosocial risk means the risk to performance created by the interaction between a person’s psychological state and their social/organisational working environment. It is not mental illness but instead the authors refer to people in the “operational middle”: those who remain medically fit and capable of doing their jobs but may nevertheless be operating under pressures such as chronic stress, fatigue, interpersonal conflict, poor leadership, job insecurity, organisational pressure, stigma around seeking help, family pressures, or a lack of psychological safety.

WHY?
Traditional aviation mental-health systems tend to concentrate on identifying individuals who have crossed a threshold into illness or potential unfitness. The authors argue this leaves an important gap between “fit to fly” and “in crisis.” Chronic stress, interpersonal difficulties, emotional strain and other psychosocial pressures can influence attention, communication, decision-making and teamwork long before someone becomes medically unfit. These should therefore be considered operational safety variables, rather than simply employee wellbeing issues.
HOW?
The authors synthesise evidence from cross-national patterns of aviation professionals avoiding or delaying psychological help and passenger perception data involving 2,772 participants. From this they propose a Five-Layer Sociotechnical Model of Aviation Mental Health, comprising technical, procedural, team-coordination, psychosocial and stakeholder-environment layers. The operational middle sits principally within the comparatively under-developed psychosocial layer where an individual’s psychological state interacts with their social and organisational environment. This includes factors such as chronic stress, psychological safety, interpersonal relationships, stigma, organisational pressures and willingness to seek support. This is the layer the authors argue aviation has not adequately incorporated into its safety architecture.
FINDINGS:
The paper proposes that psychosocial risk should be incorporated into aviation’s existing sociotechnical safety architecture rather than managed through a separate mental-health system. The authors suggest developing adaptive operational competencies that help personnel recognise and manage psychosocial pressures before they significantly affect performance. These capabilities could be incorporated into existing CRM and EBT structures.
SO WHAT?
The paper offers a useful reframing of the relationship between mental health, Human Factors and operational safety. It tries to move attention away from the binary question of whether someone is psychologically “fit” or “unfit” and towards the much larger population who continue working safely while experiencing varying levels of psychosocial load.
For SMS, it suggests psychosocial risk should be treated like other performance-shaping conditions: something to identify, understand, monitor and design around, rather than only responding once an individual reaches crisis.
